The following form can be used to submit information prior to your arrival for scheduled test, procedures and/or admission at ECM Hospital, ECM East Diagnostic Center or Shoals Hospital.

On the day of your scheduled procedure, please be sure to bring a copy of your orders. Also, please let the receptionist know that you completed your pre-registration on our web site.
