Multidisciplinary Team Approach
J.W. Sommer provides an multidisciplinary team approach which includes: daily medical physician visits; consultation with our medical director (a doctor specially trained in physical medicine and rehabilitation, called a physiatrist); 24-hour Registered Nursing care; physical, occupational, and speech therapies; case management, and nutrition and respiratory services.
Every week, a team meeting, or conference, is held with the physiatrist, case manager, rehab nurse, physical therapist, occupational therapist, and speech therapist. Each patient’s medical and functional progress is discussed, as well as the discharge plan and recommendations of the rehab team. A family member or designated caregiver is updated weekly after each team conference by the case manager.
